APPLICATION FOR SHIUR ALEF
Dear Parents,
We are pleased to inform you that registration for the upcoming Yeshiva year (5787) is now open. We look forward to receiving your son's application to Yeshivas Beis Dovid Shlomo (YBDS) Mesivta.
To complete the application process, please ensure the following items are submitted to our office as soon as possible:
1. Completed Registration Form and Fee

2. Copy of your son's most recent report card(s)

3. Teacher Assessment Form
Your son's teacher should complete the assessment form and fax it directly to the Yeshiva at: (203) 773-9237. The form must be sent directly from the school.

Applications will only be reviewed once all required documents have been received in our office. We therefore kindly ask that you submit everything promptly.
Accepted students will then be able to proceed with the next steps in the registration process.
